Introducing the Chronic Conditions Framework

​NT Health, Aboriginal Medical Services Alliance Northern Territory (AMSANT) and the NT Primary Health Network (NTPHN) have come together to launch a new framework to help prevent chronic conditions in the Territory.

Healthy, Well and Thriving (2024 – 2030) will help keep Territorians healthy and well for as long as possible by focusing on chronic conditions prevention and early intervention.

Chronic conditions, such as diabetes, cardiovascular disease, cancer and chronic kidney disease are some of the key drivers of the burden of disease in the Territory. These conditions are driven by a range of social, environmental, structural, economic, biological, cultural and commercial factors.

We are taking a systems approach to prevention and prioritising key risk factors that are driving our burden of disease: unhealthy food and drink, tobacco, sedentary lifestyles, harmful alcohol use and social and emotional wellbeing.

The framework aims to create healthy environments where we live our lives to make being healthy as easy as possible, while also reducing the progression of disease through early screening and intervention.

By working together with our key partners - Aboriginal Medical Services Alliance Northern Territory (AMSANT) and the Northern Territory Primary Health Network (NT PHN) - and other organisations and agencies across the Territory, we can be sure we are delivering health improvements for all Territorians.
